About the Online PHDS
The Online Promoting Healthy Development Survey (PHDS) is a reduced-item version of the
PHDS. This new resource provides parents and providers with an assessment tool to guide patient-centered quality improvement. The free, fifteen-minute survey assesses the quality of well-child care delivered to children under four years old. The PHDS is endorsed by the National Quality Forum.
Get Started with the Online PHDS
Parents Any parent with a child between the ages of 3 and 48 months old can take the Online PHDS! After completing the survey, you will receive an online customized feedback report to print and bring to your next well-child care visit. This report will help ensure your child receives recommended care during well-child care visits by providing suggestions to help you partner with your doctor or other health care providers to improve the care your child receives. Health Care Providers and Health Systems: Build your own Online PHDS
Step 1: Learn about the Online PHDS (print and read ALL of the following)
Step 2: Register and Customize Your Online PHDS
- Register by completing this online User’s Form: Click Here
- At the end of this User’s Form, you will be asked to create a user name and password
- You will then be directed to a password-protected site for providers www.PHDS-Toolkit.org
Step 3: Administer Your Online PHDS
- Log in to www.PHDS-Toolkit.org to review your site and survey information
- Personalize post cards and letters to notify parents of your survey
- Retrieve your feedback reports

No time to register but want to send parents to take the survey any way?
No problem, send them to www.yourchildshealthcare.org to take the Online PHDS and parents will receive a customized feedback report to help them partner in their child's care and to help guide well-child care visits. This option will not produce provider level reports, you must register and customize your Online PHDS to receive these.
What the Online PHDS Offers Health Care Providers/Systems
- Meet national recommendations for preventive services and well-child care through quality improvement measures anchored to the American Academy of Pediatrics’ third edition of the Bright Futures Guidelines.
- Obtain personalized survey administration materials, such as post cards and letters, to inform parents about your survey.
- Customize your survey by utilizing the ProPHDS, the ProPHDS-5 or select the topic-specific sections that best fit your needs.
- Receive confidential system, office and/or provider-level feedback reports.
